regardless of one’s ability to pay.

Our Sliding Fee Program offers a discounted rate on medical and dental services offered at our facilities. In addition to our Sliding Fee Program, our facilities provide convenient parking and easy access for patients with a physical disability, as well as resources in both English and Spanish, and interpretive services.

Coordinated care

Your care team will support your healthcare needs by reviewing your complete medical history, including any records of care outside of our center, to help create the best plan of care with you.

Accessible services

Patients can access the center 24 hours a day, 7 days a week by calling the main number. During regular office hours your call will be answered by a Patient Services Center representative. After hours, patients have access to the on-call provider who can assist with any urgent medical need. You can also send an electronic message to your care team at any time through the patient portal.
